In an effort to best protect the Marist community, we appreciate you submitting information about your son or daughter’s close contact with someone who tested positive for COVID-19. After submission, our school nurse, Mrs. Amanda Gaida, will be in touch with you to review protocols moving forward.  If your son or daughter has been vaccinated and your son or daughter has no symptoms OR if the close contact occurred outside, you do not need to fill out this form as your son or daughter is not considered a close contact.
